The Bureau of Inspection and Certification is making the COMPREHENSIVE APPLICATION FOR PRIOR APPROVAL REVIEW (Form OMH 165) available via the Internet in the Acrobat format.

To be able to successfully complete the application you need to have Part 551 (effective March 5, 1999) of the regulations plus the following:

                                Outpatient Projects - Parts 587 and 588
                                Inpatient Projects- Parts 580 and 582
                                Residential Adult Projects- Part 595
                                Residential Children and Youth Projects- Part 594
                                CPEP Projects- Parts 590, 591 and 596

If you do not have the applicable regulations, it is strongly advised that you contact the Bureau of Inspection and Certification at (518) 474-5570 to obtain these documents. Many of the regulations noted above may be downloaded from OMH's Bureau of Policy and Regulations page under the section "Full Text". Be advised that these documents are downloadable only in a WordPerfect format.
